Your healing, comfort and well-being are our primary concerns. If at any time you have questions, concerns or a complaint about the level of care or service provided, we encourage you to promptly bring it to the attention of any member of the care team. Patient relations staff are available by phone or to meet with you to discuss and investigate a grievance or concern that you have. Your concern will be handled in a confidential manner. Patient relations can be reached Mondays through Fridays, 8 a.m. to 4 p.m. If you would like to detail your concern in writing, you can use this site's feedback form or complete a grievance/complaint form. The grievance/complaint forms are available in public areas throughout the hospital. Listening and responding to you is one of many ways Concord Hospital strives to assure you a pleasant stay.

If your concerns cannot be resolved through our patient relations office, you can report concerns to the New Hampshire Department of Health and Human Services Bureau of Health Facilities Administration or to the Joint Commission’s Office of Quality Monitoring.
