The Concord Hospital Medical Group (CHMG) consists of more than 100 primary and specialty care physicians located throughout Concord and its surrounding communities. As hospital departments, CHMG practices have access to the most up-to-date technology, resources and educational training.

    Concord Hospital's primary care physician practices were awarded three-year recognition (beginning July 2006) from the Heart/Stroke Recognition Program, sponsored by the National Committee for Quality Assurance, the American Heart Association and the American Stroke Association. The program recognizes physicians who provide high quality cardiovascular and stroke care and motivates other physicians to document and improve their patient care delivery. This recognition comes on the heels of the Concord Hospital participation in the Diabetes Physician Recognition Program, awarded in 2004 by the National Committee for Quality Assurance and the American Diabetes Association. The Diabetes Physician Recognition Program recognizes and endorses physicians who attain quality standards based on national benchmarks. This recognition should reassure patients that their physician is providing exceptional diabetes care as recognized by a national quality organization.

  • Use of Electronic Medical Record (EMR)
    Our physician practices use electronic medical record (EMR) software, digital versions of paper records, to help deliver higher-quality, efficient health care services. The EMR provides a quick and easy means of reviewing clinical data, processing - lab and test results, and consulting with specialists.

  • Access to Concord Hospital’s Financial Assistance Program
    Concord Hospital is committed to extending financial assistance, free or reduced fee care, to qualifying individuals. If payment of your health care expenses could create a financial hardship for you, our staff will work with you to apply for financial assistance. Financial counseling is offered through Concord Hospital’s Patient Financial Services Department. Applications will be processed, and approval will be determined based on specific criteria. Information you provide is confidential and is only reviewed by staff processing your application. If approved, financial obligation to Concord Hospital will be reduced. We will not discriminate in the determination of financial assistance eligibility on the basis of race, color, creed, sex, age or handicap. You may also be eligible for financial assistance available through NH Health Access Network.

    Each of our practices retains its autonomy to make practice specific decisions while benefiting from certain benefits of a larger system. Concord Hospital provides substantial opportunities for education and increases opportunities for its physician practices to take advantage of economies of scale for purchasing activities.

  • Patient satisfaction is measured through Press Ganey
    Our practices measure patient satisfaction through Press Ganey, an independent provider of satisfaction surveys and other satisfaction measurement tools. The surveys, which are mailed to patients’ homes, ask patients to rate various aspects of their office visit on a scale of very good (100) to poor (0). The data is used to assess and improve performance of the practice.

    As employees of Concord Hospital, all our providers have access to valuable services and programs offered through Concord Hospital’s Organizational Development Department including: interpersonal skills, leadership, managing individuals and teams, and enhancing influence.
